How the lien process works with our office
- Attorney sends the referral — email [email protected] with the client’s demographics, date-of-loss, insurance carrier (if any), and a brief injury summary. Or call (562) 252-0816.
- We fax/email back our lien packet — includes the Physician’s Lien (Cal. Civ. Code § 3045.1 et seq. when applicable), Assignment of Benefits, HIPAA authorization, and standard intake paperwork.
- Initial consultation scheduled within 3–5 business days at the office most convenient to the client (Downey, Tarzana, or Bakersfield).
- Treatment plan shared with attorney — narrative reports, procedure notes, and imaging summaries sent on request. We accommodate reasonable deposition and IME scheduling.
- Lien resolution at settlement — we negotiate reductions in good faith when the settlement facts support it. Final payoff handled through the attorney’s trust account.
Why attorneys refer to Axis Pain & Anesthesia Group
- Board-certified anesthesiologist — Dr. Cohen is board-certified by the American Board of Anesthesiology with fellowship training in Pain Medicine at UCLA. Credentials that hold up under cross-examination.
- Image-guided technique — every interventional procedure is performed under fluoroscopy or ultrasound. Radiology reports and procedure imaging available for the chart.
- Detailed narrative reports — written to the standard expected by personal-injury and workers’ comp firms, with clear causation language where the record supports it.
- Rapid scheduling — new-patient evaluations within 3–5 business days; follow-up procedures typically within two weeks.
- Three Southern California offices — Downey (Los Angeles County), Tarzana (San Fernando Valley), and Bakersfield (Kern County).
- Communicative billing team — our staff responds to attorney status requests within one business day.
