Non-Discrimination Notice and Language Access

Axis Pain & Anesthesia Group does not discriminate against any person on the basis of race, color, national origin, age, disability, or sex (including pregnancy, sexual orientation, and gender identity). Free language-assistance services and free auxiliary aids are available to you.

Our non-discrimination commitment

Axis Pain & Anesthesia Group complies with applicable federal civil-rights laws and does not discriminate, exclude people, or treat them differently on the basis of race, color, national origin, age, disability, or sex (including pregnancy, sexual orientation, and gender identity). This notice is provided in compliance with Section 1557 of the Affordable Care Act (42 U.S.C. § 18116) and its implementing regulations at 45 CFR Part 92, and reflects related requirements under Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, the Age Discrimination Act of 1975, and Title IX of the Education Amendments of 1972.

What this means in practice

The practice, its physicians, and its staff:

  • Do not deny benefits, services, or participation in programs or activities on the basis of a protected characteristic
  • Do not provide a lower quality of care, use separate facilities, or treat people differently in ways that cause harm
  • Provide free aids and services to people with disabilities so they can communicate effectively with us — for example, qualified sign-language interpreters, written information in accessible electronic formats, and other formats upon request
  • Provide free language services to people whose primary language is not English — for example, qualified interpreters and written information in the language you speak

If you need any of these services, please tell us when you schedule or check in for your appointment, or call us in advance at (562) 252-0816 so we can arrange the service before your visit.

If you believe we have failed to provide these services

If you believe Axis Pain & Anesthesia Group has failed to provide these services or has discriminated in any way on the basis of race, color, national origin, age, disability, or sex (including pregnancy, sexual orientation, or gender identity), you may file a grievance with our Civil Rights Coordinator:

You may file the grievance in person, by mail, email, or phone. If you need help filing, the Civil Rights Coordinator will assist you. You may also authorize another person to file the grievance on your behalf.

You may file a civil-rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights:

Auxiliary aids for people with disabilities

If you have a disability and need help communicating with us or navigating our facilities, please let us know. Free aids and services are available, including:

  • Qualified sign-language interpreters
  • Written materials in large print, accessible electronic formats, or other formats
  • Assistance with scheduling and paperwork
  • Service-animal accommodations

Please request these services at least 72 hours in advance of your appointment when possible so we can arrange them. For in-office accommodations or emergent requests, we will do our best to respond immediately.
