This policy explains what personal information Axis Pain & Anesthesia Group collects through this website, how we use it, who we share it with, and the rights California residents have under state privacy law.

Scope of this policy

This policy applies only to information collected through the public website at axispaingroup.com. It does not cover information collected in a clinical setting — such as intake forms completed in our offices, medical charts, imaging, or billing records. Information collected in the course of care is Protected Health Information (PHI) and is governed by the Health Insurance Portability and Accountability Act (HIPAA). Our separate Notice of Privacy Practices covering PHI is provided to every patient at their first visit and is posted in each of our offices.

Axis Pain & Anesthesia Group is the business responsible for the personal information collected through this website. Contact details are at the end of this policy.

Information we collect

We collect two kinds of information through this website:

Information you provide directly

When you contact us through this site — for example, by using a contact form, emailing [email protected], or calling the phone number on the site — we collect whatever you choose to share. This typically includes:

  • Your name
  • Your email address and/or phone number
  • A brief description of why you are reaching out

Please do not include detailed medical history, diagnoses, or other sensitive health information in a website contact form or general email. Those conversations belong in a clinical setting where we can protect them appropriately. If you need to share PHI with us before your first visit, please call our office at (562) 252-0816.

Information collected automatically

When you visit this website, certain information is collected automatically by your browser and by analytics tools we use:

  • Your IP address (which can indicate your approximate geographic region)
  • Your device and browser (operating system, browser type and version, screen size)
  • The pages you visit on this site, the order you visit them in, and how long you spend on each
  • The referring website that sent you here, if any
  • Standard cookies (see the Cookies and analytics section below)

We do not ask for, and we do not want, sensitive personal information through this website — including Social Security numbers, financial account numbers, precise geolocation, or information revealing racial or ethnic origin, religious beliefs, citizenship, or union membership. Please do not send any of this through the site.

Cookies and analytics

We use Google Analytics 4 to understand how visitors find and use this website. Google Analytics sets first-party cookies in your browser (typically named _ga and _ga_*) that allow us to count unique visits and measure aggregate site behavior. Google Analytics also processes your IP address to approximate your location and to filter out automated traffic.

We have configured Google Analytics with:

  • IP anonymization (IP addresses are truncated before they are logged by Google)
  • Data retention consistent with Google’s defaults and not exceeding 14 months of event-level data
  • No Google Signals or cross-device personalized advertising tied to this property

We do not use third-party advertising pixels (Meta Pixel, TikTok Pixel, etc.), ad-retargeting tags, session-replay tools, or behavioral-profiling scripts on this website. This is a deliberate choice to keep our public site aligned with HIPAA privacy principles and to avoid introducing unvetted third-party data collection into a healthcare context.

  • Using your browser’s private/incognito mode or cookie-blocking settings
  • Using a browser that sends the Global Privacy Control (GPC) signal — which we treat as a valid opt-out request under California law

How we use this information

We use the information described above to:

  • Respond to your questions and schedule appointments
  • Send you information you requested (for example, an intake packet or directions)
  • Understand which pages and topics visitors find useful, so we can improve the site
  • Detect and prevent fraud, abuse, or misuse of the site
  • Comply with legal obligations (for example, responding to subpoenas or lawful requests)

We do not use information collected through this website to train artificial-intelligence models, to build advertising profiles, or to make automated decisions that have legal or similarly significant effects about you.

Who we share information with

We share information only with a small, fixed set of service providers, each of whom is bound to use it only for the limited purpose for which it was provided:

  • Google — for Google Analytics 4 (website analytics) and Google Workspace (email delivery for messages sent to [email protected])
  • Our electronic health record vendor — which holds the medical records we create when you become a patient of the practice, under a HIPAA Business Associate Agreement
  • Our website host — which stores the site’s files and routes HTTP requests to your browser

We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We may disclose information if legally compelled to do so (for example, in response to a subpoena, court order, or lawful governmental request), or to protect our rights, safety, or property.

No sale or sharing for advertising

Under the California Consumer Privacy Act as amended by the California Privacy Rights Act (collectively, the CCPA/CPRA), we do not sell your personal information, and we do not share it for cross-context behavioral advertising. We have not done so in the preceding 12 months.

If you visit this site with a browser that sends the Global Privacy Control (GPC) signal, we honor that signal as a valid opt-out request. Because we do not sell or share personal information for advertising in the first place, the practical effect is the same for all visitors.

How long we keep information

We keep personal information only as long as we need it for the purposes described above, or as required by law:

  • Contact inquiries (name, email, phone, reason for outreach) — retained in our email system until your question is resolved, then archived for as long as is reasonable for recordkeeping. You may ask us to delete these earlier by emailing the contact below.
  • Analytics data — retained by Google Analytics per our account settings, not to exceed 14 months of event-level data. Aggregated, de-identified reports may be kept longer because they do not identify individual visitors.
  • Server and security logs — retained for a short operational period (typically no more than 30 days) and then rotated out, except where a specific incident requires longer retention.

Separately, information collected in a clinical setting is retained under applicable medical-records laws, which can require retention for many years after your last visit. That retention is addressed in our HIPAA Notice of Privacy Practices, not here.

Your privacy rights under California law

If you are a California resident, the CCPA/CPRA gives you the following rights with respect to personal information we collect through this website:

  • Right to Know. You can ask us what categories of personal information we have collected about you, the sources, the business purpose, the categories of third parties we have disclosed it to, and the specific pieces of information itself.
  • Right to Delete. You can ask us to delete the personal information we have collected from you, subject to certain legal exceptions (for example, we may need to keep records to comply with medical-records or tax law).
  • Right to Correct. You can ask us to correct inaccurate personal information we have about you.
  • Right to Opt Out of Sale or Sharing. You can tell us not to sell or share your personal information. As stated above, we do not sell or share it; this right is provided for completeness.
  • Right to Limit Use of Sensitive Personal Information. You can tell us to limit our use of sensitive personal information to what is necessary to provide the services you have requested. We do not knowingly collect sensitive personal information through this website.
  • Right to Non-Discrimination. We will not deny you services, charge you a different price, or provide a different level or quality of service because you exercised any of these rights.

These rights are separate from your rights under HIPAA regarding your medical record. HIPAA rights are described in our Notice of Privacy Practices, provided at your first visit.

How to exercise your rights

To exercise any of the rights above, contact us by either of the following methods and tell us which right you are exercising:

To protect your information, we will ask you to verify your identity before we act on your request. The verification we ask for will be proportionate to the sensitivity of the information and the nature of the request. We aim to respond within 45 days of receiving a verifiable request, as required by California law, and we will tell you if we need more time.

You can also designate an authorized agent to make a request on your behalf. We will ask the agent for written permission signed by you, and we may still ask you to verify your own identity directly.

Children’s privacy

This website is intended for adults seeking care for themselves or a loved one. It is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through this website. If you believe a child has submitted personal information to us through this website, please contact us and we will delete it.

Third-party links

This site contains links to third-party websites, including outside informational resources. We do not control those sites and are not responsible for their privacy practices. Please read the privacy policy of any third-party site before sharing personal information with it.

Security

We use reasonable administrative, technical, and physical safeguards to protect personal information collected through this website, including encrypted transport (HTTPS) for all pages. No method of transmission over the internet or method of electronic storage is 100% secure, and we cannot guarantee absolute security. Please do not send sensitive health information through the website; call our office instead.

Changes to this policy

We may update this policy from time to time. When we do, we will change the Last updated date at the bottom of this page. Material changes will be highlighted on this page for a reasonable period before they take effect.
